How does the Alltel U Prepaid, 'Pay Per Month' plan work?

I'm a user of Alltel's Pay Per Day plan and I'm looking to change to the Monthly version. I just have a couple of questions,

1. How do you pay? Can you still pay with Refill cards?
2. What happens when you run out of Anytime Minutes?
3. How can you purchase extra things like, ( ringtones, ringbacks, etc. )
